HOW DO I...
OBTAIN REPEAT PRESCRIPTIONS?

Repeat prescriptions are issued at the doctor’s discretion and are normally for patients on long-term treatment. Requests for prescriptions must be made in writing or by completing the order form issued with your prescriptions. We take prescription requests by fax or mail but not by telephone. Please include a stamped addressed envelope if you need your prescription posted to you.

Please add a daytime contact number so that the doctor can contact you if there is any problem with your request. Prescriptions are available 48 hours after you place your request (excluding Bank Holidays and weekends).


HOW DO I...
MAKE AN APPOINTMENT?

All consultations are by appointment and can be booked from 08.40 by telephone or 24 hours via the website www.patient.co.uk
The surgery doors open from 08.50.

The surgery offers same day and pre-booked appointments, mostly of 15 minutes' duration, and we also hold late surgeries on Wednesday and Thursday evenings.

If you feel that your problem cannot wait until the next available appointment, please tell the receptionist.

The practice cares for over 10, 400 patients so there is a lot of pressure on appointments. If you cannot keep your appointment please contact the surgery in good time to cancel it so that someone else can use it.

When the doctors are running late this is usually due to either a patient needing more time or the doctor having to leave surgery to carry out an urgent home visit. Please be patient: you may require the extra time one day!


HOW DO I...
OBTAIN A HOME VISIT?

Please ring the surgery before 11.00 where possible to allow the doctors to organise their visits.
You can select home visits requests by pressing option 2 after calling the main number.


HOW DO I...
OBTAIN TEST RESULTS?

Please telephone the surgery on weekdays between 11.00 and 12.00 or between 17.00 and 18.00 to see if your results are back. Most results take two weeks. You can select results by pressing option 3 after calling the main number.

For information on insurance, medical reports and to arrange medical examinations, press option 4 after calling the main number.


HOW DO I...
REGISTER WITH THE PRACITICE?

All newly registered patients should provide NHS number, immunisation details (children under 6) and information about smear tests (women). Please make sure you keep us informed of your current address and contact telephone numbers including a mobile number if you have one. If mail that has been sent to you comes back returned, you will be removed from our practice list and your records sent back to the Health Authority.

Once registered, alll new patients will be given an appointment with the nurse or healthcare assistant to discuss their health. This provides us with important information until your medical records arrive. Undetected medical problems may be picked up during this check-up. You will need to bring a urine sample with you.

CHANGE MY DETAILS?

Please make sure you keep us informed of your current address and contact telephone numbers including a mobile number if you have one. If mail that is sent to you comes back returned, you will be removed from our practice list and your records sent back to the Health Authority.


HOW DO I...
SPEAK TO A GP OR NURSE?

If you wish to speak to a GP or nurse during a working day, please speak to one of our reception team by phone or in person. If the doctor or nurse is unavailable to take your call the receptionist will take your contact details and the doctor or nurse will call you back, as soon as possible.
